Four nights in a Roman villa or a Trastevere monastery, three in a palazzo in old Naples, five above the sea between Sorrento and Praiano. No car until the last stop, and maybe not even then.
History first, then the sea
Rome and Naples are two of the great cities of Europe and forty minutes apart by fast train; the Amalfi Coast is an hour beyond. This route is built to be done mostly by train and ferry, with a car only if you want it for the coast. Twelve days gives each stop enough time to stop feeling like a visit.

Rome, from a house with a garden
Fly into Fiumicino. Four nights is the minimum for Rome; sleep somewhere with a courtyard or a garden, because the city is loud and you will want to close a door on it.
- Forum and Colosseum on day one, early; Vatican Museums on a booked morning
- Trastevere and the Gianicolo at sunset; a Sunday at Villa Borghese
- A half-day in Ostia Antica if Pompeii is later in the trip

Naples, in the old centre
Seventy minutes by Frecciarossa. Stay in the centro storico, in a palazzo on the decumani, and eat everything. Pompeii and Herculaneum are a local train away.
- Spaccanapoli, the Cappella Sansevero, the Archaeological Museum (where Pompeii's finds actually are)
- Pompeii by Circumvesuviana, or Herculaneum for a smaller, better-preserved site
- Pizza in the old town, a night at the San Carlo opera if the season allows

Sorrento and the coast road
Ferry or train to Sorrento, then the coast. Five nights lets you split between Sorrento (easier, ferries everywhere) and the coast proper (Praiano, between Positano and Amalfi). Rent a car only if you want the inland hills; the ferry does the rest.
